Dr. Puckett-Perez's fees are within the usual and customary standard for the industry, and compare well to the fees of other clinical psychologists who have similar education, training, and expertise.
Diagnostic Testing (eg., ADHD, Learning Disabilities, Mood Disorders, testing for school or college class accommodations, etc.):

Will involve testing fee, paid on the first day of testing or split across sessions. The testing fee covers the entire assessment process, including intake interview, assessment hours, observations, test scoring, written report, and feedback session with parents and child. This testing report can be shared with your child's school district to support special accommodations in the school setting.

Testing fee for comprehensive testing: $2400. Testing also can be done in a more limited and targeted capacity, for reduced fees. Dr. Puckett-Perez can help you determine if more limited testing is a better fit for you or your child.
Insurance
This is a private pay practice, meaning that Dr. Puckett-Perez's services are not in-network with insurance panels and patients pay out of pocket at the time the service is provided. However, you will be provided with an itemized receipt (also called a "super bill") including diagnostic codes which you can present to your insurance to determine if all or part of your visit can be reimbursed to you. Dr. Puckett-Perez is happy to complete out-of-network provider forms for your insurance, or in-network single case agreements, if these are required.
Payment
Check, money order, and all major credit cards accepted for payment, including FSA and HSA cards. For testing, checks are the preferred method of payment, however, all are accepted.
Cancellation Policy
Please notify us as soon as possible when you know you cannot attend your or your child's appointment. Dr. Puckett-Perez will work to provide options for rescheduling. If you do not notify us, or "no call, no show" for your appointment, you will be charged the full fee for the session.
