Our office is conveniently located in the North Dallas/Addison area, near the intersection of Preston Rd and Beltline Rd (located at Preston and Alexis cross streets). It is in the Signature Exchange building (large building with bronze tinted windows, with "E=MC2" and "Summit Alliance" written at top of building). It is easily accessible from Dallas, Addison, Plano, Richardson, Carrolton, and surrounding areas. Parking is free and convenient. There are open 2 hr visitor spaces and a parking garage with designated visitor parking. Please take elevators to the 5th floor, and enter Office Suite 550. Please tell the front desk receptionist that you have an appointment with Dr. Puckett-Perez.

 

To make an appointment for testing services, please call our office. Dr. Puckett-Perez is often with patients and is unable to answer phone calls. However, our office maintains a confidential voicemail line that is actively monitored. If Dr. Puckett-Perez is with a patient or unavailable, please leave a detailed message including your information and a brief description of your concerns.
